Why do so many mental health words get mixed up?

Because most of them arrived in everyday speech through a side door. A word like trauma or dissociation carries a precise clinical meaning and a loose casual one at the same time, and nobody announces which one is being used. The clinical meaning is usually narrower than people expect.

Does using the wrong word actually matter?

Sometimes it matters a great deal. Physical dependence and addiction get used interchangeably, and the confusion keeps people from taking medication that would help them. A panic attack and an anxiety attack are not the same thing, and only one of them is a defined clinical event. Getting the word right is often how you get to the right conversation.
